The short answer: It depends on the drug, and each FDA label sets its own window. For a weekly shot, you can take a late dose within 5 days for Ozempic, 4 days for Mounjaro or Zepbound, and about 4 days for Trulicity. After that, skip it and go back to your usual day. Never double up, and some labels tell you to restart at a lower dose after a longer gap.
What do the Ozempic and Wegovy labels say about a missed shot?
Ozempic and Wegovy both contain semaglutide, but their labels phrase the missed-dose rule in different ways.
The Ozempic prescribing information says: "If a dose is missed, administer OZEMPIC as soon as possible within 5 days after the missed dose. If more than 5 days have passed, skip the missed dose and administer the next dose on the regularly scheduled day." After that, you go back to your normal once-weekly schedule.
The Wegovy label counts forward to the next dose instead of back from the missed one:
- If the next scheduled dose is more than 2 days (48 hours) away, take the missed Wegovy injection as soon as possible.
- If the next dose is less than 2 days away, do not take the missed dose. Take the next one on your regular day.
In practice, the two rules land in almost the same place. Both give you about 5 days of a 7-day cycle to catch up.
Wegovy adds a rule Ozempic does not have. "If 2 or more consecutive doses of WEGOVY injection are missed, reinitiate dosage escalation at a lower dosage to reduce the risk of gastrointestinal adverse reactions." The patient section of the label puts it simply: if you miss 2 or more weeks in a row, call your healthcare provider about how to restart. The Ozempic label gives no restart rule for several missed weeks.
What if you miss a Mounjaro or Zepbound dose?
Mounjaro and Zepbound both contain tirzepatide, and their labels use the same words. The Mounjaro label says to take a missed dose "as soon as possible within 4 days (96 hours) after the missed dose. If more than 4 days have passed, skip the missed dose and administer the next dose on the regularly scheduled day."
The Zepbound label repeats that rule word for word. Both Medication Guides add a firm limit: "Do not take 2 doses" within 3 days (72 hours) of each other.
Neither tirzepatide label gives a rule for restarting after several missed weeks. That decision is left to your prescriber. If you have been off tirzepatide for a while, ask before you take your old dose again. Stomach side effects are most common when a dose goes up, as covered in our GLP-1 side effects guide. For more on tirzepatide itself, see mounjaro.md.
What if you miss a Trulicity dose?
The Trulicity (dulaglutide) label counts forward, like Wegovy, but with a longer cutoff. Take the missed dose "as soon as possible if there are at least 3 days (72 hours) until the next scheduled dose."
If fewer than 3 days remain, skip it and take the next dose on your usual day. The Medication Guide adds: "Do not take 2 doses of TRULICITY within 3 days of each other." The label has no rule for restarting after several missed weeks.
What if you miss a daily Victoza or Saxenda shot?
Liraglutide is a daily drug, so the rule is simpler. Just take the next dose on schedule. The Victoza label says: "Do not administer an extra dose or increase the dose to make up for the missed dose."
The restart rule is where daily liraglutide differs from the weekly drugs. Both liraglutide labels set a hard 3-day line:
- Victoza: "If more than 3 days have elapsed since the last VICTOZA dose, reinitiate VICTOZA at 0.6 mg once daily." After that, your provider decides how fast to raise the dose.
- Saxenda: the Saxenda label says to "reinitiate SAXENDA at 0.6 mg daily and follow the dosage escalation schedule" if more than 3 days have passed.
Both labels say the reason is to lower the risk of stomach side effects when treatment restarts. Liraglutide has a half-life of about 13 hours, so it leaves the body far faster than the weekly drugs do.
What if you miss a GLP-1 pill?
Four once-daily GLP-1 pills now have FDA labels, across three labels. Their rules are not the same.
- Rybelsus and Ozempic tablets (oral semaglutide): the shared label says: "If a dose is missed, skip the missed dose and take the next dose the following day." Do not take more than 1 tablet a day.
- Wegovy tablets (oral semaglutide 25 mg maintenance dose): the Wegovy label gives the same rule. Skip the missed tablet and take the next one the following day.
- Foundayo (orforglipron): this pill works the other way. The Foundayo label says to "take the dose as soon as possible" and "not to double up the next dose." If you miss 7 or more doses in a row, the label says to restart the dose increases at a lower dose.
Semaglutide pills have strict timing rules. You take them on an empty stomach with a small sip of water and wait 30 minutes before eating. Our guide to GLP-1 pills vs injections covers how each pill is taken.
How far apart must two weekly doses be?
Sometimes a missed dose is a sign your usual day no longer works. The labels allow you to move it, with a minimum gap between the last dose and the next one:
- Ozempic: at least 2 days (more than 48 hours) between two doses.
- Wegovy injection: the patient section says your last dose must have been 2 or more days before.
- Mounjaro and Zepbound: at least 3 days (72 hours) between two doses.
- Trulicity: the last dose must have been 3 or more days before the new day.
These gaps are also the reason a late dose has a cutoff. If you take a very late Mounjaro dose, then your next dose on the regular day, the two could land too close together. The cutoff stops that from happening.
Why is a short gap tolerable at all? These drugs stay in the body a long time. The labels give a half-life of about 1 week for semaglutide and about 5 days for tirzepatide and dulaglutide. One late or skipped dose does not drop the level to zero. A longer gap is a different matter, as covered in our article on what happens when you stop taking GLP-1 drugs.
The bottom line
Check the label for your exact drug, because the windows differ: 5 days for Ozempic, 4 days for tirzepatide, and a forward count for Wegovy and Trulicity. Daily semaglutide pills are skipped, not made up. Never take two doses close together to catch up. If you have missed 2 or more weekly Wegovy doses, more than 3 days of liraglutide, or 7 or more Foundayo tablets, the label calls for a lower restart dose. For any gap longer than a week on any GLP-1 drug, talk to your prescriber before your next dose.
